~I will submit claims to said insurances for your convenience through Samantha Farina at TOTLbill(www.TOTLbill.com)  Copays are due at the time of service. 

InsightMoves PT is also a direct pay practice (out-of-network provider) for insurances not listed above..  For those insurances with which I do not contract,  I can submit the claim for you or upon request, give you a detailed receipt for you to submit to your insurance company for potential reimbursement according to their out-of- network provider fee schedule.  Payment is collected at the time of service. 

~The following are my services in detail and rates under direct pay:

Comprehensive Musculoskeletal Physical Therapy Evaluation (in-office) ~ $198

• For a complete musculoskeletal evaluation including a thorough interview, physical exam, posture and movement testing, manual assessment, initial treatment and developing your home program. 60-75 min.

Physical Therapy Follow-up visit 30-60 min ~ $125    

• For a re-assessment of the response to the initial evaluation/home program and progression of treatment toward your goals.

Videoconference Physical Therapy Evaluation~ $125

• When distance is a challenge or available time is a limitation to coming to me I provide a complete, remote video consultation including a thorough interview, posture and movement testing, initial treatment and home program. 60-75min.

Videoconference Follow-up visit 30-45 min ~ $75 

• When distance or time is the limitation to coming to me for subsequent visits I offer remote, video re-assessment visits to evaluate the response to the prior session/home program and to progress your program. 

In-home/work Visit ~ $225 evaluation/$150 follow-up  (available upon request for some locations)

• When  assessment and treatment in the home or at the work site is desired I can provide an on-site visit. 

Movement/Mobility/Strength/balance Examination (Maintenance) ~ $125 (in-office)

• For those who aren't necessarily hurting or injured but feel they're not moving as easily or as fully as they'd like to, not as strong as they'd like to be, and/or want to improve their balance, this service is available. A full body assessment to identify where your restrictions are that may be affecting your performance or creating a potential risk of injury. 60-75 min

Movement/Mobility/Strength/balance Follow-Up ~ $75 (in-office)

• improvements require some work and often don't happen overnight. The Follow-up sessions are available to guide you, progress and build on what was started on the examination. What achieves an initial positive response often has to change if you expect to achieve even more progress. Based on your response from the previous session(s) adjustments will be made to ensure maximum benefit. (Up to 50 min.

"Laser" Physical Therapy~ $75 (in office or video/phone) 

• A very concentrated professional consultation for those of you that don't necessarily need or want full physical therapy services and just want to know what's going on and what you can do about it.  (Up to 20 minutes).  This is not intended to take the place of a full, thorough evaluation. 

Vertigo  Evaluation ~$125 (in office or home/office site only)

• For a complete vertigo evaluation including a thorough interview, physical exam, special movement testing, initial treatment and developing your home program. 60-75 min.  

Physical Therapy Follow-up visit for vertigo 20-30 min ~ $70    

• For a re-assessment of the response to the initial evaluation/home program and progression of treatment toward your goals
